Contracts / terms for private classes
I seem to remember that some time ago someone posted details of their contract or terms and conditions for private lessons. I cannot remember the name of the original thread, or who posted the stuff I'm looking for ,r:; (note to self: save interesting things the minute you see them!)
Anyway - I was wondering if anyone had such a contract or agreement that they might be willing to share, or if you know of one that you could point me in the direction of?

Mine's not really a contract, per se, but and explanation of my policies for any lessons taught from my home studio.
REFUND & CREDIT POLICY
SEMI-PRIVATE/PRIVATE LESSONS
A great deal of time and effort is put into the lesson plans for Galatea Middle Eastern Dance’s Private and Semi-Private lessons. In addition, when time is reserved in advance for a class, it limits Galatea’s availability for other classes and performances. Because of this, there is a strict refund/credit policy in place regarding classes that are canceled, missed or that the student arrives late for.
The refund policy for Semi-Private and/or Private Belly Dance lessons, paid for in advance is as follows:
Cancellation after 1 class is completed
Refund of fees paid in advance, less the following:
-1 individual class session fee ($30 per person Private/$15.00 per person Semi-Private)
AND
- 10% of all advance fees paid
Cancellation after 2 classes are completed
Refund of fees paid in advance, less the following:
- 2 individual class session fees ($60 per person Private/$30 per person Semi-Private)
AND
- 20% of all advance fees paid
Cancellation after 3 or more classes are completed
No refund
The policy regarding classes that a student is unable to attend, or arrives late for is as follows:
Missed/Late Classes
Because significant time is spent preparing for your lesson, you will be charged in full for the entire hour, unless you give 24 hours notice that you will be unable to attend class.
Due to Galatea’s busy schedule, make-up classes are usually not possible. If you have given at least 24 hours notice that you will be absent, your class fee for that hour will be credited forward to the next lesson.
If you are going to be late for class due to unforeseen circumstances, please notify us by calling (262) 844-2483 as soon as possible. Due to the scheduling of multiple classes, it is often not possible to run class past your scheduled ending time, so your class time will be shortened by the number of minutes tardy.
If you are more than 15 minutes late for class, and have not called the number above, the class will be cancelled with no refund or credit given.
Galatea Middle Eastern Dance retains the right to terminate the remaining classes, without refund, if there are more than 3 absences or late arrivals per 6-week session.

I have some "rules and regulations" that I make new students read and sign - I can post it when I get home tonight. Bearing in mind that I teach private lessons in my home, the basic points are:
1. Less than 24h notice for a cancellation means you have to pay the full class fee (I frequently waive this for long time students with good excuses...but it's good to be able to cover my butt for chronic cancelers)
2. No more classes can be scheduled until you've paid up any canceled classes.
3. Do not ring doorbell prior to your lesson time, as you may interrupt another lesson. Likewise, assume that I have a lesson after yours, so if you need to use the bathroom/change after your lesson, please let me know so we can end a couple minutes early.
4. Even if you arrive late, your lesson will STILL end at the appointed time.
5. Turn off your cell phone.

this is just my general terms:
Ravena is available for private instruction. All private sessions are located in the Sterling/Herndon area unless otherwise requested (additional fees may apply). Price of private session is quoted by request.
If you would like me to choreograph a piece of music for you, the cost varies depending on length of music, complexity, and instruction. If I use my own music, additional fees apply.
Contact me with your interest so that we may discuss your needs in greater detail. If you cancel a private session less than 48 hours, you are obligated to pay half of the cost of the session. If you cancel less than 24 hours, you must pay the full session cost. Additional fees may be added on for traveling expense per each 10 miles radius.
