"Consult a doctor" "Dance at your own risk" forms - can I see yours?
Ok, I am starting up teaching again but this time - I am not in the comforts of a community center. In the past, I have always had my private students (that I have taught at other studios) sign a little sheet which states that I cannot be blamed for injuries, that you should consult a doctor before doing any exercise and blah blah blah.
Well - I lost it. I cannot find it!
I realize that this will not really protect me when it comes down to real issues but it does give all involved peace of mind.
I do have insurance so I am not trying to use this in place of insurance. .p::
Am I making sense or am I just rambling...lol

The little I remember from business law indicated that a contract is not a contract unless BOTH parties receive something from it. So I try to phrase ours like that. Also, your insurance company might be able to help you with the 'perfect' disclaimer.
'PLEASE do not do anything that your body objects to! There are so many beautiful movements in this dance that if one is not possible another can be substituted. Be respectful of your limits, enjoy yourself as a dancer, and consult your health-care professional if you have any concerns. Please enroll yourself by reading the following Disclaimer and signing below.
'I understand that there can be risks involved in dancing. In return for the educational opportunity that this Class/Workshop provides, I agree to act in a safe and appropriate manner at all times and to hold Ruric-Amari, Samovar Dance LLC, teacher-name, and venue-name harmless in case of damage, loss or injury incurred by or during my participation.'

I am just reaffirming what Maura said. We got our "Hold Harmless" form from the Insurance company. Check with them. They can probably help you. My form wouldn't be applicable to your situation, or I'd send it.

This is based on a form provided by my insurers, and expanded by me. It starts off with name & contact details + emergency number to contact
Do you have, or have you had? (Please tick box for YES)
Any heart condition
High blood pressure
Epilepsy
Diabetes
Dizziness or fainting
Are you taking any prescribed medication for a medical condition?
Are you pregnant (or trying to get pregnant)?
If you answered "Yes" to any of the above questions, it is recommended that you seek medical advice before starting dance classes.
Do you have or have you had? (Please tick box for YES)
Arthritis
Asthma
Muscular pain
Any pain or major injuries, particularly in the following areas:-
If you answered "Yes" to any of the above questions, then please discuss with the teacher before starting dance classes.
Are you in good health at the moment? Yes/No
If not, please give details .
STATEMENT
I recognise that the teacher is NOT able to provide me with any medical advice, with regard to my medical fitness, and that this information is only to be used as a guideline to the limitations of my ability to dance. I have answered the questions to the best of my knowledge, and I understand the above advice. I also undertake to advise the teacher if any medical condition develops which might affect my ability to dance.
