Yes, the outermost points of my lower back tat were definitely the most painful part... like being sliced with a razor blade.
Never having given birth or broken any bones, I'd say this was the most excruciating pain I've ever had. Not unbearable, but I decided 90 minutes is my limit. Overall, that one hurt more than my shoulder tat. I had an area about the size of a nickel on my shoulder where I didn't feel anything at all.
What's amazing is that immediately after it was finished, the pain vanished.
