I've used zazzle and been very happy with the quality. I used cafepress once years ago and wasn't very impressed (but seems their quality has increased based on the number of users). But I have used zazzle many times since. I agree with Lauren that a nice advantage to somewhere like zazzle is that it allows you to have a link to your own store or even just the one item and people can order it that way, saving you trouble and effort.

However, if I'm looking for really nice quality, I prefer screen printing and I've always gone with customink.com. They have great customer service, will work with you to make sure your design is just how you want it (example: they told me some of my type was too small to actually come out clearly. another time they recommended I invert some of my colors because of the color fabric I was printing on). And great quality of course. Oh, and pretty fast too! This only becomes cost effective at slightly larger (20+) orders, but they do have lounge/yoga pants, so if you're printing a bunch to sell it might be worth it. They also seem pretty open to combining orders if you're using the same screen (i.e., 10 shirts of one kind and 10 of another, they'll charge you the 20-shirt price for each type of shirt, not the 10-shirt price).

I've seen that customink also does individual shirt runs as well, but I've never used that service so I can't speak for it. But for their screen printing--A+.
